چکیده

Objective As the antiphospholipid syndrome (APS) is characterized by antibodies which bind negatively charged phospholipids either directly or mainly through different target epitopes located on the beta-2-glycoprotein-I ( 2GPI) molecule, the aim of this study is to describe an additional target epitope for anti-cardiolipin binding. Methods The binding characteristics of affinity purified anti-cardiolipin antibodies from a patient with monoclonal gammopathy associated with clinically overt APS were studied; inhibition studies were also carried out. These antibodies were used for the active induction of experimental APS. Results The affinity purified anti-cardiolipin antibodies were found to bind a target epitope created by the complex of cardiolipin/ 2GPI, while not reacting with a complex composed by another phospholipid (phosphatidylserine/ 2GPI), as confirmed by direct binding and competition assays. Immunization of naive mice with this unique affinity purified anticardiolipin antibody resulted in the induction of experimental APS (thrombocytopenia, prolonged coagulation timed and fetal resorptions). The anti-cardiolipin/ 2GPI injected mice developed high titers of mouse anti-cardiolipin/ 2GPI antibodies with the same binding characteristics as the human antibody which was used for disease induction. Conclusion APS is a unique syndrome that is characterized by a diversity of pathogenic anti-phospholipid antibodies which may explain the diversity of clinical manifestations reported in patients.

THROMBOSIS AND HEMOSTASIS 2 Glycoprotein I ( 2GPI) binds platelet factor 4 (PF4): implications for the pathogenesis of antiphospholipid syndrome

Antiphospholipid syndrome (APS) is an autoimmune thrombophilia characterized by arterial/venous thrombosis and/or pregnancy morbidity in the presence of antiphospholipid antibodies that mainly recognize 2 glycoprotein I ( 2GPI). To investigate potential platelet ligands of 2GPI, platelet membrane proteins from healthy persons and patients with APS were passed through a 2GPI-affinity column. By ...

Antigen Specificity of Antiphospholipid Syndrome-Related Antiphos- pholipid Antibodies

The antiphospholipid syndrome (APS) is an autoimmune disorder characterized by the presence of antiphospholipid antibodies (aPL) and clinically by vascular thrombosis and/or recurrent pregnancy morbidity. The APSassociated aPL are mostly directed against 2-glycoprotein I ( 2GPI) and prothrombin. In contrast, aPL linked to infectious disorders are primarily directed against phospholipids and do ...

Summary. Objectives: To evaluate the clinical accuracy of antiphospholipid antibody (aPL) specificities both individually and/or in combination, in a wide cohort of systemic lupus erythematosus (SLE) patients in an attempt to identify a panel of tests that may provide the best accuracy for diagnosing antiphospholipid syndrome (APS). Patients and Methods:This study included 230 patients (218 wom...

Laboratory Criteria of Antiphospholipid Syndrome Need to be Updated or Strictly Followed?

The antiphospholipid syndrome is a very relevant disease that implicates different clinical features and laboratory criteria. Regarding laboratory diagnosis, it has been established in the last consensus that tests included are lupus anticoagulant (LA), anti cardiolipin (aCL) and anti 2 glycoprotein I antibodies (anti 2GPI) of IgG or IgM isotype at medium to high titres. There is not one unique...
